You will need to bring for verification a completed documentary evidence sheet. (Download / View) and the Applicant's Information sheet (Download / View) with : -
* The name and full address of your club
* Your National Insurance Number
* Full addresses of all the places you have lived in the last 5 years. (including postcodes please)
YOU MUST BRING:
3 documents from group 1
or
3 documents: One from group 1 and two from group 2
or
5 documents from group 2

Valid Passport
Driving Licence
Birth certificate (issued within 12 months of date of birth)
Valid photo identity card (EU countries only)
UK Firearms licence
HM Forces ID card
Group 2
Marriage Certificate
Re-issued UK Birth certificate (issued after 12 months from date of birth)
P45/P60
Bank or building society statement (less than 3 months old)
Utility bill (less than 3 months old)
Valid TV licence
Credit card statement (less than 3 months old\)
Store card statement (less than 3 months old)
Mortgage statement issued within past 12 months
Valid insurance certificate
Correspondence or a document from the benefits agency: the Employment service, the Inland Revenue (HMRC) or a local authority less than 3 months old
Financial statement e.g. Pension: endowment: ISA issued within past 12 months
Valid vehicle registration document
Mail order catalogue statement (less than 3 months old)
Court summons issued within past 12 months
Valid NHS card
Addressed payslip (less than 3 months old)
National Insurance number card
Exam certificate e.g. GCSE NVQ
Child benefit book issued within past 12 months
Connexions card
Certificate of British nationality
Council tax statement
Work permit/visa issued within past 12 months
